Payment
Payment is accepted at the time of your service. You can be provided with a receipt if you wish to file a claim with your health insurance plan. Services you receive may be covered in full, or in part, by your insurance. Cash, check and all major credit cards are accepted for payment
Insurance
I am not contracted with any insurance providers and do not bill insurance. However, you may want to consider submitting your bill to insurance for reimbursement. Many health insurance plans will reimburse at least part of the fee. Each insurer pays differently depending on your contracted benefit plan. Because my private practice is not affiliated with any insurance panels, it is considered "out-of-network". If you wish to file with your insurance for reimbursement, please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per session? (see below for CPT Codes)
Professional Fees
Hourly Rate: $180
$225 per Initial Evaluation/Consultation (60-90 minutes) - CPT E/M Code 90792
$180 per follow up sessions (53-59 minutes) - CPT code 90837
In addition to treatment sessions, fees are charged, on a prorated basis, for other professional services you may require, such as: letter writing, attendance at meetings or consultations with other professionals which you may have authorized; preparation of records or treatment summaries; telephone conversations which last longer than 5 minutes; or the time required to perform any other service which you may request of us.
Cancellation Policy
Services are by appointment only. Because your appointment time is reserved only for you, it is important that you do not miss any appointments. There is a 48-hour cancellation policy for existing patients which requires that you cancel your appointment 48 hours in advance of your scheduled appointment to avoid being charged for the full session. If you do not show up for your scheduled therapy appointment, you will be required to pay the full cost of the session.
